			<content:encoded><![CDATA[<p>The necessary referral documents for Centerpoint Health&#8217;s &#8220;Montgomery House&#8221; Group Home are now available online in .pdf format.  Click on the Document Server tab above, and search the results under the section title &#8220;Housing Issues&#8221;.  The availability of these documents online should make it easier for 3rd party case workers to review the services offered by the group home, as well as speed up the process of referral for their clients in need.</p>
<div id="_mcePaste">Centerpoint Health halfway house program is a 24 hour supervised residential program located in the Pleasant Ridge/Kennedy Heights area of Cincinnati on Montgomery Rd.  It is a co-ed facility for adults aged 18 and older with severe mental disorders who have been assessed to have difficulties living independently in community either because of the extent of their illness directly or because of problems in executing the skills needed for negotiating their way in the community. The goals of the program are to provide: • Supervision necessary to ensure the safety and well-being of the individual while developing ADL skills. • Individualized and group training appropriate to the skill area identified as needing improvement for independent living. • A non-judgmental environment in which to practice the skills of independent living. • Motivation, support and encouragement to each resident to reach their personal goals associated with their quest for independent living. The average length of stay in the program is 120 days, but depends heavily on an individual’s previous experience and case management support to find affordable, appropriate housing following the Halfway House training. Key skills targeted by the program are: • Planning and preparing nutritious meals. • Budgeting and money management to be able to meet one’s obligations (rent, utilities, food). • Maintaining one’s personal health (choosing a PCP, following medical directives, providing basic self care, first aid, etc.). • Understanding the importance of developing daily activity outside of the home(work, school, support groups, volunteering, leisure activities) • Personal hygiene and good grooming. • House keeping and personal organizational skills. Centerpoint Health is also able to provide short-term respite on an available basis.</div>

<p><span style="font-size: x-small">Cincinnati—Mayor Mark Mallory and Dr. Noble Maseru, Commissioner of Health, today jointly declared that a Winter Shelter Emergency will be in effect tonight and tomorrow night and will end on the morning of Friday, December 11th. This means the Emergency Winter Shelter located at the Over-the-Rhine Community Center, 1715 Republic Street, will be open from 10:00PM until 8:00AM tonight and tomorrow. Social service agencies and community organizations should refer persons who are not accepted by existing shelters to the OTR Center.</span></p>
<div><span style="font-size: x-small"></span></div>
<div><span style="font-size: x-small">A Winter Shelter Emergency is declared when temperatures are in the single digits and/or wind chill is in the single digits for sustained periods of time; when it is determined that regular shelters are expected to be filled to capacity; and life threatening conditions could result from lack of temporary housing. In addition the Winter Shelter Emergency communication system remains activated.</span></div>
<p><span style="font-size: x-small">The Mayor’s Office, Cincinnati Health Department, Cincinnati Recreation Commission, Cincinnati Police, The Drop Inn Center, Duke Energy and the Greater Cincinnati Red Cross are collaborating to provide temporary shelter and support services.</span></p>
<p>&#8220;At these temperatures we are concerned about the health and safety of persons without adequate shelter,&#8221; said Dr. Steve Englender, Director of the Center for Public Health Preparedness for the Cincinnati Health Department.</p>
<p>The elderly, young children, adults under the influence of alcohol, and the mentally ill are some of the most at risk populations for hypothermia, which is an abnormally low body temperature that can affect the brain and make the victim unable to think clearly or move well. Shivering, exhaustion, confusion, memory loss, slurred speech and drowsiness are all warning signs for hypothermia. Frostbite, an injury to the body caused by freezing resulting in a loss of feeling and color in affected areas such as the nose, ears, cheeks, chin, fingers, or toes, is also a risk.</p>
<p>Given these extreme temperatures the Cincinnati Health Department and the Centers for Disease Control and Prevention remind you take precautions to protect yourself and loved ones from these conditions such as:</p>
<p>* Eat well balanced meals with plenty of carbs and avoid alcohol.</p>
<p>* Limit skin exposure to the extreme cold and avoid extended periods of exposure.</p>
<p>* Wear proper clothing such as a hat, scarf, sleeves that are snug at the wrist, mittens, and several layers of dry clothing.</p>
<p>* If you suspect a problem with frostbite or hypothermia you should seek medical care. If body temperature is below 95 degrees it is an emergency.</p>

			<content:encoded><![CDATA[<p>Birth certificates are required by a number of service providers to demonstrate citizenship for service eligibility.  Birth, death, and marriage records, if these events occurred within the city limits of Cincinnati, can be obtained at the Elm Street Clinic Vital Records Office.</p>

<p>The Elm Street Clinic Vital Records Office offers same day service up to 2 p.m. Monday through Friday.  Wait time is generally one hour.  They open at 8:30 a.m. except on the 1st Tuesday of each month, at 10:30 a.m.  If you do not have a receipt or a picture ID matching the certificate request, your certificate will be mailed to you.  Any requests for vital records made from 2:01 p.m. through 4 p.m. will be mailed to the customer.</p>
<p>Effective 10/16/2009 the cost of all certificates is $27 (formally $22).  Types of payment accepted include: cash, certified check, money order.  Some credit and debit cards are now accepted, as well (Check and see if the card says &#8220;Interlink&#8221; on it &#8212; if so, it is part of a network of service providers NOT accepted by the Vital Records Office at the Elm Street Clinic).  There is an extra $2.50 charge if you pay using one of the accepted credit or debit cards.  They do not accept $100 bills, or personal checks.</p>
<p>Vital records are available only for those events which took place within the geographic limits of Cincinnati.  Institutions within the city of Cincinnati include: Good Samaritan Hospital, Catherine Boodth Hospital, General/University Hospital, The Christ Hospital, Providence (Franciscan-Mt Airy Campus) Hospital, Jewish Hospital (Clifton), Veterns Medical Center, Drake Center, Bethesda Oak (Maternity), St. Frances/St. George (Franciscan Western Hills Campus), Our Lady of Mercy Hospital up to 1951, and approximately 175 nursing homes.</p>

You will be asked to submit a name, cell phone carrier, and cell number, as well as to agree to the terms of service (e.g., you pay for all the text messages you receive, at the rate specified in your cell phone contract).  By subscribing to the SMS services, you will receive a text message whenever the Interagency Bulletin Board is updated.</p>
<p>The feature is fully operational but will be considered &#8220;Beta&#8221; until we receive a certain level of feedback from our users to evaluate the functionality.  In the future we would like to have SMS functionality available independently for each cybercasemanager.com localization.  Check it out, and let us know what you think.</p>
